Agreements & Forms (submit all forms to your advisor)

Open a DCS precious metals account:

• Precious Metals Account Opening Form (Required for all accounts)
• Precious Metals Clearing Account Agreement (Required for all accounts)
• If you wish to designate a beneficiary upon your death, also complete:
• Transfer on Death Form (If applicable)
• If you are opening a Trust account, also complete:
• Trustee Certificaton Form (If applicable)
• Client Resolution Form (If applicable)

To perform one or more of the following special tasks:

• • Send a wire to a bank or financial instituon that is not listed in the account records
• • Prepare a check made payable to third party (certain restrictions apply)
• • Send a check to an address that is not listed in the account records
• • Ship bullion to an address that is not listed in the account records
•    Complete the following: Letter of Authorization
